We are privileged to work alongside people who believe, like we do, that locally owned and operated healthcare facilities should go beyond simply "meeting the needs" of its residents.POSITION SUMMARY:The responsibilities of the MMQ Coordinator includes but is not limited to overseeing and coordinating of developments and completions of the member assessment (MMQ) in accordance with current Federal and state regulations, coordinating and leading the development of a comprehensive member assessment and care plan for each customer.Duties of the MMQ Coordinator include but are not limited to:* Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulations and facility policies.* Monitors completion of MMQ by other disciplines within timeframes prescribed by regulatory guidelines.* Advises supervisor of incomplete and/or untimely assessments by disciplines other than nursing.* Complete all Admission, Conversion, Significant Change and Semi-Annual MMQs per timeframes outlined in MMQ instructions as updated by the Office of Long Term Care.* Initiates care plans and supporting activities that will result in best possible outcome for assigned residents.Responsibilities and requirements for MMQ Nurse Coordinator:* Valid RN or LPN License in the state of MA* Minimum of 2 years of experience in a long-term care setting, preferred.* Clinical experience, education or specialty skills specific to geriatrics is desired.* Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long term care.* Great interpersonal skills for effective daily interaction with Residents, family members and all members of the Interdisciplinary Team* Must have knowledge in PPS assessment as well as OBRA compliance.* Must be a self-starter and a high attention to accuracy and details and possess good time management skills* Ability to make independent decisions and follow through with limited supervision.* Must have at least 1 year of MDS/MMQ coordinator experience.Next Step Healthcare is an equal opportunity employer.
