Job Description:

Encompass Health is seeking a dedicated Case Manager to enhance the quality and effectiveness of patient care within our integrated healthcare services. Based in one of our esteemed facilities across 42 states or contributing through our expansive home-based care system, the Case Manager will be pivotal in managing patient recovery plans and facilitating interdisciplinary approaches to care. The ideal candidate will excel in a fast-paced, dynamic environment and be adept at balancing compassionate patient interactions with rigorous healthcare management protocols. Develop patient care plans in collaboration with healthcare teams. Monitor patient progress and adjust care plans as needed. Ensure resources are utilized effectively and adhere to healthcare regulations and standards. Advocate for patients' needs and act as a liaison between patients, families, and healthcare providers. Document and maintain accurate records of patient interactions, treatments, and outcomes. Strong organizational and communication skills. Ability to manage multiple cases simultaneously without compromising quality of care. Proficient in healthcare management systems and electronic health records. Medical Insurance Dental Insurance Vision Insurance 401k Licensed Registered Nurse (RN) or Licensed Social Worker (LSW), depending on specific role focus. Certification in Case Management (CCM) preferred.
