Job Description:
The LPN role involves providing compassionate nursing support to
patients and families in Newton and surrounding communities,
assisting with assessments, care planning, and comfort measures in
accordance with the Interdisciplinary Plan of Care.
Preferred experience in hospice, home health, or geriatrics;
requires at least one year of relevant experience.
Responsibilities include communicating with RN Case Managers,
coordinating care, maintaining accurate records, and educating
caregivers on medication, personal care, mobility, safety, and
end-of-life preparation.
Participates in team meetings, supports families at the time of
death, and provides 24/7 coverage as needed.
Qualifications include a current LPN license, valid driver--------s
license, ability to pass background checks, and a commitment to
hospice philosophy and confidentiality.
Must demonstrate teamwork, integrity, and proficiency in pain
management through ongoing learning.
